HOW TO PREPARE FOR YOUR APPOINTMENT
Please arrive 20-25 minutes prior to your appointment to complete the registration process. To prepare for your appointment, please bring the following:
- Driver’s license or photo I.D.
- Insurance card(s)
- A list of your current medications
- Any x-rays, MRI, or CT scan films and reports that have been done at an outside facility (other than Riverside, Sentara or Mary Immaculate. We are able to view these online).
- Referrals
Depending on the type of insurance you have, you may be required to obtain a referral to our practice. Please bring this on the day of your appointment or contact your primary care physician to have the referral faxed to our office. If your referral has not been received in our office prior to your appointment, we may request that you complete a financial waiver.
