Policies & Procedures at Hampton Roads Orthopaedics Spine & Sports Medicine

CO-PAY AND OUTSTANDING BALANCES POLICY

Effective August 1st, 2020, all Hampton Roads Orthopaedics Spine & Sports Medicine offices will start collecting insurance co-pays at the time of check-in. In addition, outstanding patient collection balances will also be required and collected at the time of check-in. Both, co-payments and collection balances will be required before being seen or the appointment will have to be rescheduled.”

PRESCRIPTION RENEWAL POLICY

Medication refill requests are accepted during regular office hours per our office policies. When requesting medication refills, be sure to allow 48 hours (two business days) notice. Please provide the spelling of your name and your date of birth as well as the name of the medication, dosage, how often you take it, the pharmacy name and phone number, and where we can reach you if the nurse has a question. Check with your pharmacy before going to pick up your prescription. You may also request a prescription renewal online.

MEDICAL RECORDS

If you are interested in obtaining copies of your medical records, you may complete your request by downloading our medical records request form. Please send the completed form (signature required) to request@bhsconnect.net or fax to (513) 653-0778. For questions about your request or invoice, please call BHS Connect directly at (513) 898-1044. Nominal fees will apply.

Please note that we retain medical records for ten years. Records of minor children shall be maintained until the child reaches the age of 18 or becomes emancipated with record retention of ten years from the last patient encounter regardless of the age of the child.

ADMINISTRATIVE REQUESTS

We will complete disability, Family Medical Leave, mortgage, or loan insurance forms, and other administrative requests for a nominal fee, which is payable in advance. Requests are processed in the order in which they are received and may take 3-5 business days for processing.
