Appointments
Our courteous and well-trained appointment representatives are available to schedule an appointment from 8:00 a.m. to 5:00 p.m.
To prepare for your appointment, please bring the following:
- Driver’s license or photo I.D.
- Insurance card(s)
- A list of your current medications
- Any x-rays, MRI, or CT scan films and reports that have been done at an outside facility (other than Riverside, Sentara or Mary Immaculate. We are able to view these on line).
Referrals
Depending on the type of insurance you have, you may be required to obtain a referral to our practice. Please bring this on the day of your appointment or contact your primary care physician to have the referral faxed to our office. If your referral has not been received in our office prior to your appointment, we may request that you complete a financial waiver.
Registration
To prepare for the registration process, please make sure to have your demographic information and insurance cards available.

Registration in the office on the day of your appointment:
Please arrive 20-25 minutes prior to your appointment to complete the registration process.
Pre-Registration:
Our registration specialist will call you 1-2 days prior to your appointment or you may call between the hours of 9:00 a.m.-4:00 p.m.
